Modified Story Memory Technique

The most common cause of memory failure occurs due to breakdown in the encoding stage of creating a new memory. The modified Story Memory Technique (mSMT) is a technique developed and researched by the Kessler Foundation to treat memory deficits, specifically targeting the encoding stage, following brain injury. Using context and imagery, their research and implementation of this technique have shown to have significant improvements with a person’s functional recall as well as an increased activation of different parts of the brain.

Morning ADL Training Session/OT Shower Room Signs

Memory Log Template

This memory log is designed for rehab therapists working with people who have memory impairments. The first of the 2 page template captures names of therapists, doctors, therapy goals, events leading to hospitalization and current location. The second page can be filled out each day the patient is in the facility and captures what was accomplished with each therapy discipline, appointments, medical notes, visitor notes, medication tracking, and general reminders.
