Morning ADL Training Session/OT Shower Room Signs

Often times an occupational therapist will schedule a shower or morning ADL session with an individual and find out the person is already dressed or had a shower from staff. This handout offers 2 room signs that can be posted to alert staff not to complete the activity, and provide a visual reminder for the patient.

Modeling Language with AAC

Memory Log Template

This memory log is designed for rehab therapists working with people who have memory impairments. The first of the 2 page template captures names of therapists, doctors, therapy goals, events leading to hospitalization and current location. The second page can be filled out each day the patient is in the facility and captures what was accomplished with each therapy discipline, appointments, medical notes, visitor notes, medication tracking, and general reminders.
