Continuum of Post-Acute Rehabilitation Care
After a life-altering neurological event, it is common for a person to require additional nursing and rehabilitation care following their acute hospitalization stay. Amongst receiving other education, family members must make important post-acute decisions based on their loved one’s needs. This handout describes five different levels of post-acute rehabilitation care: long-term acute care hospital, inpatient rehabilitation facility, skilled nursing facility, home healthcare, and outpatient, what type of person would benefit from each level, and average length of stay.
