This screener provides you with information regarding the discrepancy between what family members are seeing and what patients are seeing as related to recent changes in cognition. Includes one form for the patient to fill out and one form for family/friends/caregivers to fill out. The document contains definitions for clinical terms for reports as well as tips on how to use the screener within the context of a rehabilitation setting.

Cognitive Recovery After COVID-19

There are still so many unanswered questions about how the COVID-19 virus affects some people after recovering. There are some known neurological and cognitive symptoms that can last long after being infected with COVID-19. Working with a speech-language pathologist can improve cognitive changes with attention, concentration, short-term memory, word-finding, and cognitive fatigue.

Cognitive Impairment Care Staff Communication Form

Therapists can fill out this form in under two minutes and share with physician, nursing, social services, psychological, and therapy staff to ensure adequate understanding of individual cognitive impairments. The form includes quick scoring boxes for the MOCA, MMSE, SLUMS, and a blank area for other test results. It includes quick checkboxes to identify memory, orientation, and problem solving/sequencing abilities. It also contains room for individualized care notes.

The IDDSI website provides a lot of helpful information for standardizing the testing of consistencies for solids and liquids; however, it can be overwhelming to locate all of the information needed for a full understanding. This resource provides all of the information required to understand each type of test, which test is appropriate for each level, and what the test should reveal in one easy-to-read chart. This can be used by dietary, nursing staff, speech-language pathologists, or other staff members or caregivers to determine if a person is being provided the correct recommended consistency.
